From what I see it appears that 5T have more elastic waist bands ect maybe they are easy to work for younger children? But I know that in my kids 5T appears to be the same size as regular 5. One more thing that I noticed was 5T appear to be cheaper since they are in the toddler department once you go to regular sizes they clother gets more and more expensive. No Size 5 and 5T are both different sizes, however some places care 5xs for boys, which seems to be pretty close to the 5T size. I understand, I too have a boy who just turned 4 last month. He is in the 5T or 5xs size as well. Good luck, but really try the 5xs, those fit better than the 5T, plus some of them have little adjustable waist in the 5xs.
I buy most of my son's clothes from Old Navy. For shirts, a size 5 in boys Old Navy is quit a bit wider and broader through the shoulders than the children's 5T, but not much longer.
